We’re looking for a qualified, experienced Health and Adults Social Worker with substantial knowledge of adult social care legislation, principles and best practice. You will also need to be currently registered with Social Work England to join us.
The ideal candidate will be committed to working creatively the within the CRSTAT (Community Rehabilitation short term assessment team) using The Strengths Based and Person-Centred Approach to enable our residents to experience personalised, integrated care. This role also requires you to support residents in your professional practice to deliver a high quality, robust, efficient and effective re-habilitation service that maximises individual user outcomes by enabling independence of residents in collaboration with their family/carer’s and partnership organisations to support a strengths-based approach that minimises the need for long term services.
This role also support residents within CRSTAT (Community Rehabilitation short term assessment team) remit to maintain their independence and wellbeing through timely assessments of their needs, implementation/review of packages of care, completing carer’s assessments, working in collaboration with our health and social care partners to cope with change, attain the outcomes they want, understand and manage risks in relation to social inclusion in the community and to facilitate and review care home placements where required. You will have experience of the assessment of needs of adults and the management of risks, including in relation to Safeguarding enquiries.
You’ll be responsible to the Team Manager and be able to fully contribute towards the improvement and efficiency of the Team.
