We have an exciting opportunity for an experienced Social Worker to join our Community Team for People with Learning Disability (CTPLD)! 

This is a full-time, permanent role with hybrid working, and a salary of £40,192 - £44,012 per annum, plus an annual retention bonus of £3,000 subject to eligibility.

The role:

You will be working as a Social Worker principally to support young people in transition from Children's to Adult Learning Disability Services. 

Your role will involve:

  • Located within a multi-disciplinary community learning disability team you would be supporting young people with learning disabilities and their families and carers to make a positive transfer to adult services. This would include liaising with Children's Services, Health Services, Education, Care Agencies and any other organisations or individuals involved. 
  • You would be carrying out assessments, undertaking care reviews, supporting care planning, and making funding applications.
  • You would take part in a duty rota to deal with incidents, safeguarding alerts and information requests.
  • You would work as a Social Worker to protect and promote the interests of vulnerable people.
  • Undertake safeguarding investigations, Deprivation of Liberty Safeguards assessments, capacity assessments and best interest decisions for adults with a learning disability.

What we are looking for:

  • A qualified Social Worker with a qualification such as a BA, BSC Diploma or equivalent.
  • Current registration with Social Work England.
  • Experience of working in a health and social care environment. The ideal candidate would have experience of working in children’s and adult services.
  • The nature of a transition post is that the work is fast paced, often complex and sometimes contentious. You would need to be able to work on your own initiative to tight deadlines while balancing contesting interests.
  • To successfully manage in this role, you would need to have regularly, independently, completed Community Dols applications, Assessments of Need, Continuing Health Care Applications, Service planning and funding applications.
  • Effective management of resources would need to be combined with a commitment to ensuring the best for the young person and their family.
  • Good understanding of the Care Act and other relevant legislation.
  • You would need to work closely with others.
